Pinnacle Hospital offers a bountiful array of benefits to our valued staff.  Our insurance benefits/employer contributions are among the best in the region!!! Unless otherwise noted, benefits become available on the first day of the month after the date of hire and are offered to Regular staff members working thirty (30) or more hours per week.

Health Plan:

United Healthcare (UHC) Choice Plus PPO program which gives you the freedom and access to a network of physicians, facilities, and other healthcare professionals, including specialists without designating a Primary Care Physician or without having to obtain a referral.

UHC offers its members some added features we are excited about: a FREE Biometric Health screening - confidentially get your glucose and cholesterol readings at no cost to you from the convenience of your own home; a Fitness Reimbursement credit of up to $240 a year for going to particpating gyms or the YMCA!  It PAYS to stay fit!!

Pinnacle Hospital pays 90% of the staff member cost and 70% of the added cost when you choose to add dependents onto the coverage.

Prescription Plan:
United Healthcare Pharmacy Program offers a broad network of retail pharmacies, as well as a home delivery option that has considerable cost savings.

Dental Plan:

Met LIfe PPO offers your choice of dentists, no waiting periods once enrolled, and no deductible or copays for preventive exams, cleanings, etc.  Other services are covered at 80% and 50% depending on the type of service.  Great news for parents: Orthodontia is included, up to 50%!

Vision Plan:

VSP Choice plan offers a VERY affordable and convenient plan.  Plan members receive a yearly exam for $10, prescription lenses or contacts yearly, frames every other year, and discounts for laser vision correction and specialty items.

Life & Accidental Death and Dismemberment Insurance:

Met Life is provided at one times the annual earnings up to a maximum amount of $100,000 PLUS an additional flat  $15,000 policy for all eligible staff members.

Pinnacle Hospital pays 100% of the staff member cost.  Additional Voluntary Life insurance and dependent coverage is available at good rates.

Short & Long term Disability Insurance:

Standard Insurance company is provided to eligible staff.  Short term disability begins on the 15th day of disability and can last up to 13 weeks. Long term disability begins on the 90th day and can last until permanent disability.

Pinnacle Hospital pays 100% of the staff member cost.

401(k)
Lincoln National offers an employee contribution program with over 50 pre-tax investment options.  Eventually, a matching program will be available and staff members would become 100% vested after three years of employment.

Paid Time Off (PTO) Program:

Pinnacle Hospital offers a bank of paid time off that includes vacation, holiday, and personal time off as well as an extended illness benefit (EIB) for staff members working twenty four (24) hours per week or more, which begins to accrue immediately upon hire.  These accruals are available for use after successful completion of the ninety day introductory period.  The accruals listed below are based on 80 hours per pay period, those working 48-79 hours per pay are pro-rated according to hours worked.

PTO includes holidays, vacation time, and personal time off and accrues as follows:

1 - 3 years of service   6.77 hours per pay   176 hours / 22 days per year
4 - 6 years of service   7.69 hours per pay   200 hours / 25 days per year
7 - 9 years of service   8.62 hours per pay   224 hours / 28 days per year
10+ years of service    9.23 hours per pay   240 hours / 30 days per year  

EIB includes illnesses lasting longer than five scheduled days and accrues as follows:

1+ years of service       1.54 hours per pay     40 hours / 5 days per year
